I understand my spray tan contains DHA (Dihydroxyacetone), which temporarily colors only the outermost layer of skin.

I understand that spray tanning is a cosmetic service and does not provide protection from UV rays or replace sunscreen.

I understand that results may vary based on skin type, hormones, medications, skincare products, preparation, aftercare, swimming, sweating, and other individual factors.

I understand that SolFlower Sunless Studio cannot guarantee an exact shade or the longevity of a spray tan.

I understand that temporary cosmetic bronzer may transfer to clothing, bedding, towels, or upholstery before the first rinse.

I understand that allergic reactions and skin sensitivities, while uncommon, are possible.

I certify that I have disclosed all relevant known allergies, medical conditions, medications, and skin sensitivities.

Liability Waiver

I authorize my child to receive spray tanning services from SolFlower Sunless Studio.

I have completed the health questionnaire and liability waiver accurately on behalf of my child.

I understand that every precaution is taken to provide a safe and professional service.

I acknowledge there are inherent risks associated with cosmetic spray tanning, including but not limited to allergic reactions, skin irritation, uneven color development, temporary staining of fabrics, and dissatisfaction with cosmetic results.

To the fullest extent permitted by Ohio law, I release and hold harmless SolFlower Sunless Studio, its owner, employees, and representatives from liability arising from this cosmetic service, except in cases of gross negligence or willful misconduct.
